Super Service is pleased to provide you with the opportunity to sign up for Medical, Dental, FSA, Vision, Accident, Short-Term Disability, Long-Term Disability, and Life insurance during this year’s enrollment period.
You will enroll by calling in to the call center beginning Tuesday, November 13th - Monday, November 19th to make your elections for 2019, 8:00am – 8pm Eastern Time.

  • To enroll you will be asked questions that will require verification that you have received and reviewed the two documents with this notice, linked below.  The enrollers will not proceed with enrollment if not acknowledged.

  • Dependent Verification is now required for any medical enrollments in 2019.  This must be completed by end of the enrollment period or insurance will not be provided to dependents.


To employees enrolled in Allstate benefits.  Your policies will terminate as Super Service, LLC enrolled benefits as of 12/31/18.  You will receive a notification after termination from Allstate if you would like to continue your benefits on an individual basis with the same rates.  You would pay the premiums directly to Allstate, instead of having premiums payroll deducted.

Long-Term Disability insurance provides you with a monthly cash benefit, after your claim is approved, to help pay for everyday expenses (such as mortgage/rent, utilities, childcare, or groceries) if a covered disabling illness or injury (e.g., back injuries and some serious illnesses such as heart attack, cancer, or stroke) takes you away from work for an extended time.
