Super Service is pleased to provide you with the opportunity to sign up for Medical, Dental, Vision, Accident, Short-Term Disability, Long-Term Disability, and Life insurance during this year's enrollment period.
Open Enrollment will be held from Tuesday, November 13th – Monday, November 19th to make your elections for 2019.
Very important: **Current elections will NOT be automatically transferred to 2019. An election must be made to keep benefits**

NEW: If enrolling spouse/dependents you will be required to submit verification documentation for your spouse/each dependent. Watch for additional details on how to submit your documents.
Medical insurance may help to pay the expenses associated with normal doctor visits or emergency treatment. You also have access to a network of medical providers.
Dental insurance can help lower your out-of-pocket expenses so you and your family can maintain healthy smiles—and better overall health, too.
Vision insurance helps keep your eyes healthy so you can stay productive at work. Eye doctors can detect signs for health conditions such as diabetes and high blood pressure before symptoms arise. Coverage can include savings on eye exams, eyewear, and discounts on laser vision correction.
Accident insurance can help minimize the financial impact by providing a cash benefit if you, your spouse, or your child experiences an injury, treatment, or loss due to a covered accident.
Short-Term Disability insurance provides you with a weekly cash benefit, after your claim is approved, to help pay for everyday expenses (such as mortgage/rent, utilities, childcare, or groceries) if you are unable to work for a short time due to a covered disabling injury or illness (e.g., back injuries, recovery from surgery, or even maternity leave).
